This is a portrait and wedding contract; please read carefully and sign below.
I, the undersigned, recognize that I am hiring the services of Nola C. Dorman, owner of Creations by Nola-LLC. I understand that I, the customer, will be responsible for all sales tax on the agreement price of this contract as well as all reorders. I agree to pay 1/3 or 1/2 (please circle), of the agreed upon amount before the wedding as a non-refundable deposit and I realize the other 2/3 or 1/2 (please circle) is due two weeks before the wedding. Upon signature and receipt of deposit of 1/3 or 1/2, Nola C. Dorman reserves the time and date agreed upon.
I also understand that I will not hold Nola Dorman or Creations by Nola-LLC responsible for lost or damaged materials due to conditions beyond their control; for example, problems with the postal system. The negatives/digital files shall remain the property of Nola C. Dorman/Creations by Nola-LLC and I understand that pictures made from the negatives and digital files may be used for display or advertising. Creations by Nola does promise to take the utmost care in dealing with negatives, film, digital files and the storage of the said items. If, for any reason, a problem does occur, liability will be limited to return of all deposits made. I, the undersigned, also understand that I will be responsible for any and all collection fees and/or attorney’s fees on any unpaid balances. I understand that Nola C. Dorman/Creations by Nola LLC is not responsible for any printing errors completed by anyone but Creations by Nola and companies utilized by Nola; most notably Millers and Digilabs.
NOTICE OF COPYRIGHT: It is ILLEGAL to copy or reproduce these photographs elsewhere and violators of this Federal Law will be subject to criminal and civil penalties. This applies to Anise and Embrace packages and any reprints ordered through Creations by Nola LLC.
The agreed upon price shall includes of the following:
Pictures of the ceremony Pictures of the reception/dance
Posed Pictures before and/or after the wedding
*Hours of time Proof Catalog with images
DVD with high or low resolution images Online Internet Proofing

8x10’s 5x7’s
*Please Note:If time is exceeded by 1/2 hour an additional charge will apply*
The Specialty Album, 8x10’s and 5x7’s and parents albums for the Elizabethan, Marquise, and Christen packages must be ordered within 100 days of the date of the wedding. If not ordered within the 100 days, an additional fee will apply. Additional orders above and beyond the contract may be placed at any time.
